Penicillin Shortage has San Diego County Rationing the Only Approved Treatment for Syphilis During Pregnancy
While San Diego County is seeing an increase in congenital syphilis cases, Pfizer recently announced a shortage of the only product approved to treat syphilis during pregnancy.
For months, San Diego County’s public health department has been following the situation and issued two CAHAN San Diego health advisories; the first advisory provides guidance and recommendations for prioritization of Bicillin L-A, and the second advisory provides an estimated recovery timeframe to be in the second quarter of 2024.
The San Diego Union-Tribune recently reported on the shortage and interviewed Dr. Winston Tilghman.
The County recommends Bicillin L-A supplies be prioritized for pregnant people and for those who are allergic to doxycycline, an alternative antibiotic used to treat syphilis in non-pregnant people.
“If we were using Bicillin L-A like we normally use it without the sorts of restrictions that we’ve been recommending, we would probably be out by now,” Tilghman said. “With the prioritization of Bicillin L-A for certain clinical situations, we are stretching it quite well; we’re going through just a couple of doses per week on average here at the county.”
“If everyone follows those guidelines that we’ve released, I think we can get through much of this without any major issues.”
San Diego County, following a multi-year nationwide trend, is experiencing a surge in the number of reported syphilis cases. According to county records, there were 38.1 “early” syphilis cases per 100,000 residents in 2021 compared to 18.3 per 100,000 in 2012.
A consequence of that rise has been the increased number of cases among newborns. Based on preliminary data provided by the County of San Diego, there were 36 in 2022 compared to just four in 2012. This article was a follow up to a recent San Diego Union Tribune article on the latest case counts of congenital syphilis, which have been increasing rapidly over the past 10 years.
Diagnosing and treating syphilis in pregnancy and ensuring that partners are tested and, if needed, treated for syphilis are best practices for congenital syphilis prevention. Both are critical in keeping babies safe, since pregnant people can be reinfected with syphilis, even after appropriate treatment, if their partners have untreated infection.
